Photocopy of the Birth Certificate
Two (2) Recommendation Forms accomplished by Class Adviser, Principal or Guidance Counselor. Note: Please use the DLS-Canlubang Recommendation Form and High School Scholastic Records.
Three (3) 2X2 photos
Testing Fee of P500 for Filipinos; USD50 for Foreigners

Additional Requirements for Transferee

Transcript of Records with Grading System or Certified True Copy of Grades in lieu of the HS Scholastic Record
Certificate of Transfer Credential or Honorable Dismissal

Photocopy of the Alien Certificate of Registration (ACR)
Special Study Permit (SSP) for those below 18 years old
Photocopy of Passport
