Admission Requirements - Integrated School

• Duly Accomplished Application Form. Click this to download form
• NSO Authenticated Birth Certificate.
• Photocopy Baptismal Certificate.
• Photocopy Report Card for the past and current grade/year level.
• Two(2) Recommendation Form accomplished by any teacher; Principal or Guidance Counselor. Note: please use the DLS-Canlubang Recommendation Form.
• Guidance Certificate of Transfer Form accomplished by the Guidance Counselor.
• Three(3) 2x2 photos.
• Testing Fee of Php 500.00

Additional Requirements for Foreign Applicants

• Alien Certificate of Registration(ACR) or Special Study Permit(SSP)
• Photocopy of Passport

Age Requirement

• Kinder 1 applicants must be at least 4.6 years old by June 1, 2011
• Kinder 2 applicants must be at least 5.6 years old by June 1, 2011

Grade Requirement General Average of 85 with no grade below 80 in any subject and in Conduct/Deportment
